Student Immunization Requirements
国内学生要求
- 的 伊利诺伊州 requires all full-time university students to submit 要求 immunization records. - Please submit a copy of your full immunization record. Military records, official medical records 和 past school immunization records are acceptable -OR- have your health care provider complete 和 sign the 2024-2025 Student Immunization Form (PDF) with verification of immunization dates.
- Please submit your records via our secure 病人门户. Alternatively, records can be dropped off at the 健康中心 (please use the drop box on the west side of the building if we are closed) or can be faxed to 773-298-3906.
- Often the easiest way to find your records are to request them from your high school. CPS students must request their records 在线. If you do 不 have your records, please download the guide Tips for Finding Immunization Records (PDF).

COVID-19接种疫苗
- It is highly recommended that all students 和 employees remain 最新的 with their COVID-19 vaccines, but it is 不 要求.
- Effective July 12, 2022, 伊利诺斯州 Governor J.B. Pritzker rescinded the requirement that higher education employees 和 students be fully vaccinated against COVID-19 or submit to weekly testing in order to access the university/college premises. 伊利诺斯州 Executive Order 2022-16 rescinds this prior requirement, 和 no further weekly testing 这个时候需要什么.
- International students who have received a vaccine 不 approved by the World Health Organization may receive an FDA-approved vaccine series at the SXU 健康中心.
International Student Requirements
- International student immunization requirements are nearly the same as domestic students; there is only the addition of the tuberculosis screening. Failing to turn in your records will result in your registration being blocked for 接下来的学期.

豁免
学生 who are medically unable to receive certain immunizations must have a 医疗豁免表格(PDF) signed by a healthcare provider. 学生 who have strongly held religious beliefs precluding them from receiving certain immunizations must sign the 宗教豁免表格 (PDF) 和 must attach an originally written, signed statement that specifies the religious belief that conflicts with receiving immunization(s).
Forms can be uploaded to our secure 病人门户, faxed to 773-298-3906 or dropped off at the 健康中心. 一旦收到 they will be reviewed by clinical staff at the 健康中心.
